a) Complete the Required Fellowship Form online at: http://www.pharmacology.med.umn.edu/ (Please
use the paper form only if you are unable to apply online: download

b) Personal Statement of Goals (view the Pharmacology
Graduate Program Diversity Statement)
c) Curriculum Vitae (include work history and any research/teaching
experience)
d) Copies of transcripts from each undergraduate or graduate
institution you have attended (they need not be official)
or a record of the courses you have completed and the grades
you received.
e) Arrange to have three letters of
recommendation sent on your behalf (list names of your
references in the appropriate field on the required
fellowship form). Your references should be familiar
with your work and able to appraise your probability
of
success
in
our program. There is no special form for the letters
of recommendation. Your references need simply
write a letter on the stationery of their institution,
addressed to the Director of Graduate Studies. Do NOT use the online letters of recommendation system when completing your Graduate School Application. Letters must be sent from the recommender to the Pharmacology Department.
f) Your scores on the Graduate Record Examination (GRE)
general test. Official scores should be sent to institution
code: 6874, department code: 0216. Please
include a photocopy of your score notification to be used
until ETS sends the official reports. The Pharmacology
Graduate Program accepts paper-based and computer-based
GRE scores for exams taken within the past 5 years.

IMPORTANT NOTE for International
students from the People's Republic of China, Hong
Kong, Taiwan and Korea:
The GRE is comprised of two steps:
a computerized analytical writing test and a paper
verbal/quantitative exam. You must take
the GRE analytical writing test between July and
September in order to have scores reported in time
for Fall. The paper test is given in October. You
must register to take the paper GRE exam before September.
